Only where the words are rarely if ever spelled out in full. (Almost) no-one writes of "general practitioners"; they write of "GPs"; -- ditto -- with "Bachelor of Medicine" ("BM"), "Member of the Royal College of Obstetrics and Gynaecology" ("MRCOG") and so on. I did gloss "MD" because, being Latin, it is slightly less obvious than the other abbreviations.

Well, if I had a complaint about something I'd had from a T/A, and went back to complain in person to the person who had prepared it, I wouldn't ask the front-of-house man if I could speak to the cook; I would ask if I could speak to the chef. To my mind, it is a matter of politeness and respect.
